Observation

Child has Individual Health Care Plan for a food allergy but it is incomplete. The medication authorization form is missing and the Benadryl. The Individual Health Care Plan requires the signature of the parent. When the medication form is completed it will require the signature of the doctor and parent. Medication authorization form needs to be present and completed for the Benadryl and Epipen.
